Both CPAP and BiPAP are positive-pressure devices used to treat sleep-related breathing disorders and lung disease. They look similar but are clinically different devices and are not interchangeable. This guide explains the differences and when each one is indicated.
CPAP: continuous pressure
CPAP stands for Continuous Positive Airway Pressure. It delivers a single, constant level of pressure throughout the respiratory cycle, keeping the airway open and preventing the typical collapse of obstructive sleep apnea.
When it is prescribed:
- Mild to moderate obstructive sleep apnea (most cases).
- Patients with apnea-hypopnea index (AHI) between 5 and 30/h.
- When the sleep study shows a single pressure solves the events.
Pros: simpler device, lighter masks, lower cost. Cons: some patients find "exhaling against pressure" uncomfortable.
BiPAP: two pressure levels
BiPAP (also written BPAP or VPAP) stands for Bilevel Positive Airway Pressure. It delivers two distinct levels of pressure:
- IPAP (Inspiratory Positive Airway Pressure): higher pressure during inhalation.
- EPAP (Expiratory Positive Airway Pressure): lower pressure during exhalation.
This makes breathing easier because the patient exhales against lower pressure.
When it is prescribed:
- Severe sleep apnea unresponsive to CPAP.
- Obesity hypoventilation.
- COPD with hypercapnia.
- Chronic respiratory failure (neuromuscular diseases).
- Patients needing very high pressures (where CPAP is uncomfortable).
Pros: better tolerance, real ventilatory support, ideal for high pressures. Cons: more expensive, more sophisticated devices, masks can be more complex.

Do I need a titration first?
Yes. Both CPAP and BiPAP require a titration, either with overnight polysomnography in a lab or with an auto-titrating device. The titration sets the correct pressure. Without that data the therapy may be ineffective or uncomfortable.
Masks: nasal, pillows, or full face
- Nasal: covers only the nose. Most popular and comfortable.
- Nasal pillows: very lightweight, ideal for claustrophobic patients.
- Oronasal / full-face: covers nose and mouth. Required for mouth-breathers or high pressures.
