How to Choose a Wheelchair for an Older Adult in Costa Rica

Choosing the right wheelchair for an older adult can mean the difference between independence and dependence, comfort and pressure ulcers, daily use or abandonment. This guide explains the most-used types, sizes, and materials in Costa Rica so you make the right call the first time.

Wheelchair types

Standard wheelchair

The most common. Steel or aluminum frame, fixed armrests, large rear self-propulsion wheels. Good for occasional use and short distances.

Transit chair

Lighter, small wheels, not self-propelled (requires a caregiver). Ideal for taking the patient to the doctor, supermarket, or church. Fits easily in a trunk.

Semi-orthopedic chair

More comfortable frame, padded seat, fold-back armrests, removable footrests. Recommended for daily use by an older adult.

Full orthopedic chair

Adjustable backrest height and recline, trunk supports, headrest, postural control. Indicated for patients with limited mobility or who spend many hours in the chair.

Bariatric chair

Reinforced frame and wider seat (up to 56 cm or more). Supports up to 200 kg. Essential for patients with significant overweight.

Power wheelchair

Motor and battery. Ideal for patients with cognitive autonomy but motor limitation. Greater independence, but requires storage and charging space.

What size to pick?

The most important measurement is seat width:

  1. Measure the patient's hip width while seated.
  2. Add 2 cm on each side (4 cm total) for clearance.
  3. That is the minimum seat width.

Standard sizes in Costa Rica: 16", 18" (most common for an average adult), 20", 22", and 24" (bariatric).

Seat depth: from buttock to popliteal fossa (behind the knee) minus 5 cm. A chair that is too deep cuts off circulation; too shallow does not support the thighs.

Common mistakes

  1. Buying an extra-wide chair "for comfort": increases lateral fall risk and makes self-propulsion harder.
  2. Ignoring chair weight: an 18 kg chair is too heavy for an older caregiver loading it into a car.
  3. Skipping the cushion: many hours without an anti-bedsore cushion creates pressure ulcers in weeks.
  4. Not testing footrests: they must sit at the right height to avoid thigh tension.

Must-have accessories

  • Gel or air cushion (anti-bedsore): for daily use > 4 hours.
  • Pelvic safety belt: prevents slipping.
  • Wheel guards: avoid clothes brushing the wheels.
  • Side bag: water bottle, phone, meds.
  • Waterproof poncho: essential in Costa Rica.

Buy or rent?

Rent when:

  • Use will be a few months (post-op, recovery).
  • The patient may improve and stop needing the chair.
  • You want to try different models before buying.

Buy when:

  • Use will be permanent.
  • The patient is already adapted to a chair specifically configured for them.
